  REGISTRATION FORM
  To book a stateroom on this sailing, you must submit a completed registration form with deposit. Request registration form from your travel consultant.

  For PC users the PDF form can be completed online, saved and returned via email to receptionist@foodandwinetrails.com.

  For MAC users, the process for properly filling in and saving a PDF form is slightly different than on a Windows machine. By default, the Mac’s Preview application is set to open all PDF files, but Preview will not save the data you put into the forms. Right-click on the pdf or hold down the Control key on your keyboard and click on it. A menu will appear, select “Open With”. Choose Acrobat Reader from the list of programs. From within Acrobat you will be able to fill in the fields and save the file with your text and submit the completed form via email.

  PAYMENTS:

  If you are paying for the deposit with a credit card, include your card information on page two of the registration form and FAX to (707) 526-9147.

  BOOKING INCENTIVES: Please note that cruise line offered promotions including but not limited to free gratuities, shipboard credits, and internet packages can be cancelled or discontinued by the cruise line without notice at any time. F&WT includes such promotional offers on this web site in good faith and will remove expired, cancelled or discontinued offers within 24 hours of notification. F&WT will not be financially responsible for expired, cancelled, or discontinued cruise line promotions they in good faith advertise on this web site as valid.

  OLIFE PROMOTION
  Standard terms apply. OLife Choice amenities are per stateroom, based on double occupancy and subject to change. Free Internet is one per stateroom; Owner’s, Vista & Oceania Suites receive access for two devices per suite. OLife Choice free shore excursions vary by voyage and exclude Oceania Choice, Oceania Exclusive and Executive Collection. Voyages up to 9 days receive 4 free shore excursions or $400 shipboard credit; 10-12 days receive 6 free shore excursions or $600 shipboard credit; 14+ days receive 8 free shore excursions or $800 shipboard credit. If shore excursion amenity is selected, all excursions must be chosen at least 14 days prior to sailing. Food & Wine Trails optional shore tours do not qualify as Oceania’s OLife Choice free shore excursions. OLife Choice beverage package amenity is House Select. Guests in the same stateroom must choose the same OLife Choice amenity and amenity must be chosen by final payment. Not all promotions are combinable.

  REQUIRED DOCUMENTATION
  Please note, Oceania Cruises requires that all guests submit a completed Guest Registration Form before final payment can be processed.
